As a full-time teacher in the Wapakoneta Area Schools, I am eligible to enter the Wapakoneta Area Community Foundation 2008 Outstanding Educator Grant competition. My principal and I understand and agree to the conditions of my receiving funding.

 
  • That I have read the "Grant Program Criteria" and completed the proposal as outlined and used by the WACF selection committee in rating my "special project"; that a maximum of $5,000 will be awarded to each of the two (2) proposals scored highest by the committee.
  • That as a full-time teacher, I accept responsibility for designing, implementing, evaluating, fairly reporting project results and accurately maintaining funding expenditures.
  • That I agree to share information about my project with other staff and to allow planned observation by the WACF members during the "special project" timeline.
  • That I am also willing to present a summary and budget report at the Wapakoneta Area Community Foundation 2009 Annual Meeting about the results of my project.
  • That all materials, equipment, creative work (tangible or intangible, acquired or developed through the grant funding) will become the property of the Wapakoneta Area Schools.
  • That I agree to remain employed as a teacher in the Wapakoneta Area Schools for a period of two years following the completion of my "special project".
  • That after completion of my "special project", I am not eligible to apply for an additional grant for a period of three (3) years.
  • That, if funded, I am to be the named recipient of a maximum of $5,000; that I must account and document all project expenditures; that I am personally responsible for returning unspent funds of $1 or more to the WACF by October 2, 2009; and if my project exceeds the amount of the grant funding, no additional funding will be available from the WACF.

Any information about the grant released to the media and /or public will credit the Wapakoneta Area Community Foundation Outstanding Educator Grant Program.
